This also reflects my patient online learning portfolio                       CASE HISTORY PATIENT DETAILS: A 75 years old female patient presented with CHIEF COMPLAINT: Swelling of lower limbs and face puffiness since 9 months HISTORY OF PRESENT ILLNESS: Patient was apparently asymptomatic 9months ago then she developed edema in lower limbs bilateral and puffiness of face. CASE HISTORY : A 60 year old female from miryalaguda  came with complaint of difficulty in breathing  CHIEF COMPLAINTS : difficulty in breathing since 15 days  Decreased urine output .since 15 days HISTORY OF PRESENT ILLNESS :  *Patient was apparently asymptomatic 3 weeks back, then she felt  difficult in breathing  *Patient came with decrease urine output since 15 days back  *Patient is  radiating pain on right side of neck during dialysis PAST ILLNESS : *Hypertension  *Diabetic *Patient got severe difficulty breathlesness  4weeks back  on relief ,she was taken to hsptl PERSONAL HISTORY :  Vegetarian diet.
